Job summary

La Frontera Center Inc. — EMPACT is seeking a Children's Case Manager at our Maricopa, AZ location to provide direct crisis intervention, support, and coordination of in-home services for families and children.

Responsibilities include needs assessments, referrals, and interdepartmental communications to ensure continuity of care. A bachelor's degree in behavioral health or related field is preferred, along with AZ driver license and fingerprint clearance; bilingual Spanish is a plus.

Qualifications

• Bachelor's degree in Behavioral Health related field or Behavioral Health experience preferred.
• Course of Study: Psychology, counseling, and social work, or related field.
• Experience with the Child and Family Team process required.
• Must have Fingerprint clearance card or be eligible to obtain one.
• Must have AZ driver's license with clean record in past 3 years.
• Bi-lingual Spanish preferred; Diff and Rural Diff per hour available.
• Must be at least 21 years old.

Responsibilities

• Provide direct service crisis intervention, support, transportation, and education to families and children.
• Complete family needs assessments and referrals to community services.
• Coordinate communications with other departments and partners.
• Ensure continuity of care and coordination of in-home services.
• Flexible scheduling to meet client and family needs.
