Job summary

Horizon Health Services in Lockport, NY, seeks a Targeted Case Manager to support patients with behavioral health needs. You’ll assess Social Determinants of Health, assist with rapid access to care, and provide outreach via calls, telehealth, and in-person visits to remove barriers and connect to community resources.

Qualifications

• Minimum education and related experience as stated in the job posting.
• Experience in a behavioral health setting with patient-facing roles is preferred.
• Familiarity with reporting and entry-level data analytics is a plus.

Responsibilities

• Complete Social Determinants of Health (SDOH) assessments for patients.
• Participate in Open Access to assist patients with rapid access to care.
• Provide patient support through outreach calls, telehealth, and face-to-face visits to identify barriers and connect patients to resources.
• Deliver transitional care with follow-up outreach to connect patients to providers.
• Connect individuals with medical, social, housing, and other supportive services.
• Assist patients in addressing gaps in care and achieving health goals.
• Retrieve and utilize data from EHR systems and health information networks.
• Build positive relationships with individuals with behavioral health disorders and their families, respecting diverse backgrounds.
• Learn and adhere to Horizon Health Services policies, including privacy regulations.
