Job summary

The Floating Hospital in Long Island City, Queens, is seeking a full-time Case Manager to join its Behavioral Health team. This role manages a patient caseload addressing mental health, substance use, life skills, and comprehensive health needs.

Responsibilities include outreach, intake assessments, coordinating referrals, documenting in the EHR, supporting crisis interventions, and collaborating with providers across community organizations to ensure continuity of care.

Qualifications

• Bachelor’s Degree preferred with two years of relevant experience.
• Direct service experience, especially in crisis intervention, case management, housing, and substance abuse.
• Must be comfortable in a high-volume environment and possess strong telephone skills.
• Bilingual in English and Spanish preferred.

Responsibilities

• Outreach referrals to assess behavioral health needs, evaluate risk, determine eligibility, and schedule intake appointments.
• Coordinate same-day referrals and work with primary care to ensure timely engagement.
• Attend weekly multidisciplinary meetings to discuss patient care and improve continuity of services.
• Maintain referral and emergency transfer logs with compliant documentation.
• Document case management activities in the EHR in a timely, accurate manner.
• Assist with crisis response protocols and care coordination during behavioral health emergencies.
• Coordinate referrals for higher levels of care and specialized services.
• Conduct monthly roster reviews to keep provider information current.
• Reach out to patients with missed appointments and reschedule as needed.
• Collaborate with healthcare providers, hospitals, schools, and community organizations to coordinate care.
• Provide scheduling support when BH Scheduler is unavailable, verify insurance eligibility and PCP details.
• Process pharmacy requests as authorized by psychiatric providers.
• Perform additional administrative and care coordination tasks as assigned.
