Job summary

Horizon Health and Wellness, Inc. in Arizona is seeking a Care Manager to ensure clients receive coordinated, compassionate services and to advocate for their needs within the care team.

The role involves documenting plans, coordinating with clinical staff, and managing a caseload in a fast-paced setting. A HS diploma with behavioral health experience and CPR/First Aid certification are required; evenings availability and a valid driver’s license are needed.

Qualifications

• HS diploma or GED plus 4 years in Behavioral Health or related education/experience.
• One year case management experience strongly preferred.
• Ability to create plans and summaries based on client needs.
• Must obtain and maintain fingerprint clearance card and pass drug/background checks.
• Must be at least 21 years of age and have a valid driver’s license with reliable transportation.

Responsibilities

• Coordinate and deliver case management and advocacy services to clients.
• Ensure client caseload is provided with necessary services in a coordinated and effective manner.
• Maintain communication and coordination with clinical team participants.
• Willingness to work evenings as needed.
