Job summary

Buffalo Federation of Neighborhood Centers is seeking a Health Home Care Manager to provide person-centered case management for adults with chronic medical or mental health needs. You will coordinate care plans, liaise with clinics, and support clients to improve health outcomes and reduce emergency services usage.

The role requires a NY driver’s license, the ability to work independently, and strong communication and organizational skills.

Qualifications

• High School degree with 3 years of experience in human services or healthcare.
• Bachelor's degree welcomed but not required.
• Valid New York State driver’s license.
• Excellent written and oral communication with strong organizational skills.
• Proficiency in Microsoft Office and ability to use electronic health records.

Responsibilities

• Care Planning and Coordination: assess and plan with clients who have chronic medical or mental health needs.
• Build person-centered relationships to support care plans and motivation.
• Care management activities including meetings, mailings, calls with clients and families.
• Manage caseloads around 55–65 clients and respond to inquiries promptly.
• Outreach and crisis intervention: engage clients, assess risks, and implement crisis plans.
• Link clients to medical, financial, social, rehabilitation, and psychiatric services.
