Connections Health Solutions is seeking a Case Manager to coordinate admissions, develop individual treatment plans, and manage discharge planning in collaboration with the interdisciplinary team.

Responsibilities include obtaining collateral information, coordinating with community providers, and maintaining compliant EHR documentation. A Virginia behavioral health background is essential, with QMHP/LMHP credentials highly valued.

Competenze

• Bachelor’s degree in behavioral health or related field is required.
• QMHP and LMHP credentials are valued.
• In-depth knowledge of Virginia behavioral health system and resources is essential.
• Must pass Virginia DBHDS/DBHDS background checks and related clearances.
