Horizon Health and Wellness, Inc. is seeking a Care Manager to coordinate services for behavioral health clients. The role focuses on advocacy, case management and ensuring timely access to necessary supports within a coordinated care plan.

Applicants should have a high school diploma or GED with 4 years in Behavioral Health, plus case management experience and CPR/First Aid certification. Evening shifts may be required, and a valid driver’s license is essential.

Formación

• HS diploma or GED plus 4 years in Behavioral Health or combined education and experience to meet Behavioral Health Technician qualifications.
• One year case management experience strongly preferred.
• Certification in first aid, CPR and Crisis De-escalation.
• Must be at least 21 years of age and have a valid driver’s license.
• Prior experience in the behavioral health setting is strongly preferred.
• Willingness to work evenings as needed.
• Demonstrated organizational abilities to maintain communication and coordination with clinical staff.
• Must obtain fingerprint clearance and pass background checks; TB screening required.

Responsabilidades

• Care Managers ensure client caseloads receive necessary services in a coordinated and effective manner.
• Provide caring and compassionate delivery of case management, advocacy and supportive services.
• Coordinate services for clients and maintain connections with the clinical team.
