Job summary

Horizon Health Services in Buffalo, NY is seeking a Clinical Care Specialist to join our mental health and substance use team. You will coordinate care, work with multidisciplinary clinicians, and respond to crisis calls at the clinic.

This full-time, on-site role offers a supportive environment, structured training, and opportunities to engage patients and families from diverse backgrounds. You’ll use an EHR and participate in quality improvement initiatives.

Qualifications

• Bachelor’s degree with 1 year of Behavioral Health or related experience required.
• Associates degree with 2 years of Behavioral Health or related experience required.
• Associates degree with at least 1 year of Horizon Health site experience required.
• High School Diploma with 6 years of Behavioral Health or related experience required.
• Experience in a computerized office environment with word processing, databases and spreadsheets.
• Ability to relate to diverse patients and families and interact positively.

Responsibilities

• Provide care coordination by relaying medication statuses and following up on releases, paperwork, and status update reports.
• Work closely with multidisciplinary clinical teams.
• Conduct toxicology tests with patients.
• Respond to crisis calls and walk-ins at the clinic.
• Review and flag patient records for timely updates.
• Monitor data via EHR, dashboards, and reports.
• Support patients with behavioral health and medical follow-ups.
• Attend Clarence Treatment Court weekly.
