Job summary

Horizon House, Inc. seeks a full-time Intensive Case Manager in its Behavioral Health Services Division to ensure clients receive essential supports to live, learn, work, and socialize in the community.

The role focuses on coordinating services, developing intake assessments, and maintaining accurate documentation to support individualized rehab plans and crisis intervention, with emphasis on teamwork and independent action.

Qualifications

Bachelor's degree in Human Services or related field.
3 years of mental health direct care experience or equivalent.
Strong communication and documentation skills.

Responsibilities

Provide continuous flow of services in compliance with contract and regulations.
Ensure delivery and accessibility of appropriate services for clients.
Develop and refine individual rehabilitation plans with goals across social, educational, vocational, self-care & residential skills; complete functional assessments.
Identify gaps in service delivery and recommend action plans.
Assist in crisis resolution and identify resources for immediate intervention.
Submit accurate documentation on a timely basis per funding source regulations.
