Job summary

Healthfirst is seeking a Pharmacy Technician to act as the member's MTM navigator, reviewing medication lists, coordinating refills, and communicating with providers to improve adherence. The role emphasizes collaboration with Healthfirst pharmacists and external partners to deliver integrated care.

The candidate will perform health assessments, document care delivered, and respond to member needs both telephonically and in person when required.

Qualifications

Clear, concise written and verbal communication required.
Experience communicating with patients, providers and pharmacists on medication issues.
Nationally certified Pharmacy Technician (PTCB/NHA) or licensed/registered technician.

Responsibilities

Review medication lists to support adherence and reduce barriers.
Facilitate refills, new prescriptions, appointments and access barriers.
Coordinate MTM interventions with pharmacists and partners.
Monitor and communicate member adherence with community pharmacies.
Conduct patient health assessments to identify barriers to adherence.
Handle member and provider communications telephonically or in person as needed.
Perform additional duties as necessary.
