Healthfirst is seeking a Pharmacy Technician to act as a Pharmacy Navigator, reviewing medication lists to improve adherence and close gaps in therapy. The role includes facilitating refills, coordinating prescriptions, and documenting care to support optimal outcomes.

You will collaborate with Healthfirst pharmacists and external partners to implement MTM plans, monitor adherence with community pharmacies, and perform patient health assessments to identify barriers.

Qualifications

• High School Diploma or GED from an accredited institution.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Pharmacy Technician certification or license in good standing.

Responsabilités

• Performs review of medication lists to understand and eliminate barriers to medication therapy as a Pharmacy Navigator.
• Facilitates refills, new prescriptions, appointments, and financial access to medications while documenting care.
• Partners with pharmacists and partners to coordinate MTM interventions within broader care management.
• Monitors and communicates patient adherence with community pharmacies.
• Performs patient health assessments to identify barriers to adherence and access to care.
• Handles member and provider communications telephonically; in-person may be required.
