Job summary

Healthfirst is seeking a Pharmacy Technician to act as a Pharmacy Navigator, reviewing medication lists and removing barriers to adherence. You will obtain refills, coordinate new prescriptions and appointments, and communicate plans with healthcare partners.

Qualifications

• High School Diploma or GED from an accredited institution.
• Experience with clear verbal and written communication.
• Experience communicating with patients, providers and pharmacists on medication issues.
• Nationally certified Pharmacy Technician (PTCB, NHA) or licensed Pharmacy Technician in good standing.

Responsibilities

• Review medication lists to improve adherence and reduce barriers.
• Assist members with refills, new prescriptions, appointments, and financial access.
• Collaborate with Healthfirst pharmacists and partners to coordinate MTM interventions.
• Coordinate with community pharmacies to monitor member medication adherence.
• Conduct patient health assessments to identify barriers to adherence and access to care.
• Handle member and provider communications by phone (in person where required).
• Perform additional duties as needed.
