Job summary

Healthfirst is seeking a Pharmacy Technician to act as a member’s Pharmacy Navigator, reviewing medication lists to enhance adherence and reduce gaps in therapy while preventing adverse events.

You will coordinate refills, facilitate new prescriptions, and assist members facing financial barriers to access medications, aligning refills to minimize pharmacy visits and documenting care delivered.

Qualifications

• High School Diploma or GED required.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Nationally certified Pharmacy Technician or licensed Pharmacy Technician in good standing.

Responsibilities

• Review medication lists to understand barriers to therapy and serve as the member's Pharmacy Navigator to improve adherence and prevent adverse events.
• Facilitate refills and new prescriptions, set up appointments, and assist financially challenged members to access medications.
• Coordinate MTM interventions with Healthfirst pharmacists and external partners as part of broader care management services.
• Monitor member medication adherence with community pharmacies and communicate relevant details to care teams.
• Perform patient health assessments to collect medication profiles and identify barriers to adherence.
• Handle member and provider communications telephonically (in person when required).
• Perform additional duties as necessary.
