Oak Street Health in Fort Wayne, IN is seeking an Outreach Executive I to build relationships with Medicare seniors and convert them into patients within the local community. You will work with Outreach Managers, CRM, and other team members to drive new patient growth in your assigned territory.

The role requires strong communication, prioritization, and collaboration skills, with a focus on event-based outreach and lead development.

Formación

• Fluency in Spanish, Polish, Russian, or other languages spoken by communities served.

Responsabilidades

• Sales: Generate leads by engaging Medicare seniors, attend meetings to learn updates, prospect and cold call, nurture prospects via email/phone, collaborate to drive local lead generation.
• Relationship Management: Maintain touchpoints with prospects, log activities in CRM, be a point of contact for clinical/center teams, address concerns and route to appropriate Oak Street Health staff, perform other duties as assigned.
