Job summary

Oak Street Health is seeking an Outreach Executive I to engage Medicare seniors and convert them into patients within the local community. You will work with Outreach Managers and Directors to drive new patient growth through active community engagement.

The role emphasizes relationship-building, prospecting, and efficient use of CRM tools. A passion for serving diverse communities and US work authorization is required.

Qualifications

• Minimum US work authorization.
• Fluency in Spanish, Polish, Russian, or other languages spoken by communities we serve.

Responsibilities

• Develop relationships with Medicare seniors to engage Oak Street Health.
• Drive new patient growth in assigned territory.
• Generate leads through community events and outreach.
• Maintain CRM and track sales activity.
• Collaborate with CRM and Outreach teams to maximize lead generation.
• Handle inquiries and help prospective patients navigate next steps.
