Job summary

Best Home Health Providers, Inc. is seeking a Medical Social Worker to lead, mentor a team of social workers, and set care standards as patients transition home from medical facilities.

This per diem role offers $100-$120 per visit, with the potential to convert to full-time based on performance, and a focus on compassionate, high-quality service across the organization. You will oversee care coordination, ensure documentation meets ACHC and state requirements, and collaborate with the Executive

Qualifications

• MSW from an accredited program.
• An active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build strong knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.
