Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker to lead the social services team in home-based care.

You will oversee care coordination, guidance, and mentorship of social workers while ensuring compliance with ACHC standards and state regulations. The role is per diem with a rate of $100-$120 per home visit, offering a flexible schedule and potential transition to full-time based on performance.

Qualifications

• MSW from an accredited program.
• Active California LCSW license.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community.
• Self-directed, organized, and flexible—take initiative and collaborate with others.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build strong knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.
