Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker to lead our social services team in home-based care, mentor staff, and set high standards for compassionate service.

You will oversee care coordination, ensure regulatory compliance (ACHC, state) and contribute to program quality while working in a supportive, family-oriented atmosphere. The role is per diem with a rate of $100-$120 per visit.

Qualifications

• Master of Social Work (MSW) from an accredited program.
• Active California LCSW license.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and interpersonal skills.
• Self-directed, organized, and collaborative.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referral sources to support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and identify new ways to better serve patients.
• Perform other duties as required for patient services.
