Job summary

Best Home Health Providers Inc. seeks a Medical Social Worker for leadership in home-based care. Lead the social services team, mentor staff, and uphold high-quality standards in alignment with ACHC and state regulations.

You will oversee care coordination, budgeting input, and the development of staff skills while maintaining patient-focused service across our Bay Area network.

Qualifications

• Master of Social Work (MSW) from an accredited program.
• Active California LCSW license.
• Minimum 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and relationship-building skills.
• Self-directed, organized and flexible with initiative.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures and care coordination with other clinical providers.
• Recruit, onboard and mentor social workers with clinical supervision and staff development.
• Oversee assessment, planning and evaluation of services, ensuring regulatory-compliant documentation.
• Partner with the Executive Director on budget and resource planning.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and ongoing education to keep skills current.
• Contribute to quality improvement initiatives and identify new ways to serve patients.
• Perform other duties as required for patient services.

As a Medical Social Worker with Best Home Health Providers, you will shape how we care for patients in their homes, mentor a team of social workers, and help set the standard for compassionate, high-quality service across our organization.

Please note that this role is per diem with a rate of $100 - $120 per visit.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program

• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
