Job summary

Best Home Health Providers Inc. is seeking a Medical Social Worker to lead our social services team in home-based care across the Bay Area. This per diem role offers clinical leadership, mentorship, and the chance to set the standard for compassionate, high-quality service.

A Master of Social Work and an active California LCSW license are required, with at least 3 years of management experience in home care preferred. The role carries a per-visit rate of $100–$120.

Qualifications

• Master of Social Work (MSW) from an accredited program.
• Active California license as a LCSW.
• At least 3 years of management experience in home care preferred.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.
