Job summary

Best Home Health Providers, Inc. in California is seeking an experienced Medical Social Worker (per diem) to lead our social services team in the Bay Area. You will mentor a team of social workers, oversee assessments, and coordinate care with clinicians to support patients at home.

This role offers per diem visits at $100–$120 each, with opportunities to convert to full-time based on performance. A Master of Social Work and California LCSW licensure are required, along with at least three years

Qualifications

• MSW from an accredited program required.
• Active California LCSW license required.
• Minimum 3 years management experience in home care.
• Knowledge of healthcare regulations and ACHC standards.
• Strong communication and interpersonal skills.

Responsibilities

• Lead daily operations of the social services team and ensure care coordination with clinical providers.
• Recruit, onboard, and mentor social workers with supervision and professional development.
• Oversee assessment, planning, and evaluation of social services with compliant documentation.
• Partner with the Executive Director on budgeting and resource planning.
• Build knowledge of community resources and connect patients to support.
• Plan in-service training and continuing education for team skills.
• Contribute to quality improvement and new service initiatives.
• Perform other duties as required for patient services.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team's skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

Requirements

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
