Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker (MSW) to lead our social services team in California, guiding care coordination and ensuring compliance with ACHC standards.

You will recruit, mentor, and supervise social workers, manage performance, and collaborate with the Executive Director on budget and operations. This per-diem role offers flexible scheduling and the potential to convert to full-time based on performance.

Qualifications

• MSW from an accredited program and CA LCSW license.
• Minimum 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and teamwork.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

Please note that this role is per diem with a rate of $100 - $120 per visit.

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
