Premier Home Health Providers Inc. is seeking a Medical Social Worker (MSW) to lead our social services team in California, ensuring compliant, high-quality care for home-based patients.

You will recruit and mentor social workers, oversee assessments and care planning, manage budget planning with the Executive Director, and coordinate with providers and community resources to support patients and families in their transition home.

Qualifications

• MSW from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and ability to build trust with patients and families.
• Self-directed, organized, and collaborative approach.

Responsibilities

• Lead daily operations of the social services team, including policies and care coordination.
• Recruit, onboard, and mentor social workers with clinical supervision.
• Oversee assessment, planning, and evaluation of social services care and documentation.
• Partner with the Executive Director on annual department budget and resources.
• Build knowledge of community resources and connect patients to support.
• Plan in-service training and continuing education for the team.
• Contribute to quality improvement initiatives and patient service enhancements.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.

• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
