Premier Home Health Providers Inc. is seeking a Medical Social Worker to lead our social services team in managing patient transitions from facility to home.

You will recruit, mentor, and supervise social workers, ensuring comprehensive care planning and high-quality documentation across the organization. You will collaborate with the Executive Director on budgeting and resource planning while expanding connections with community resources to support patients and referrals.

Qualifications

• Master of Social Work from an accredited program.
• Active California license as an LCSW.
• Minimum 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community.

Responsabilités

• Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.
