Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker Manager to lead the social services team in a home health setting. You will mentor staff, oversee care coordination, and ensure high-quality service across the organization.

The role requires an MSW and an active California LCSW license with at least 3 years of management experience in home care, plus strong regulatory knowledge. Per diem engagement with a pay rate of $100–$120 per visit.

Qualifications

• MSW from an accredited program.
• Active California LCSW license.
• Minimum 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.

Responsibilities

• Lead daily operations of the social services team, including policies and care coordination with clinical providers.
• Recruit, onboard, and mentor social workers with clinical supervision and growth support.
• Oversee assessment, planning, and evaluation of social services care and ensure compliant documentation.
• Partner with the Executive Director on annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and continuing education to keep skills sharp.
• Contribute to quality improvement initiatives and improved patient care.
• Perform other duties as required for patient services.
