Job summary

Caring Hand Home Health Solutions, Inc. is seeking a Medical Social Worker to lead our social services team in California and mentor staff to deliver compassionate in-home care.

The role focuses on policy development, care coordination, and superior patient support within a multidisciplinary setting. You will supervise social workers, ensure compliant documentation, and collaborate on department budgeting while building connections with community resources and referral sources.

Qualifications

• MSW from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.
• Excellent communication skills and ability to build trust with patients and families.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build strong knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services
