Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker to lead our social services team in California on a per diem basis. You will mentor staff, ensure compliant documentation, and coordinate care for patients transitioning from facilities to home settings.

Required are an MSW from an accredited program and an active California LCSW license, plus at least 3 years of management experience, preferably in home health. A flexible schedule and strong regulatory knowledge are essential.

Responsibilities

• Lead daily operations of the social services team, including policies and care coordination.
• Recruit, onboard and mentor social workers with clinical supervision.
• Oversee assessment, planning and evaluation of social services care with proper documentation.
• Partner with the Executive Director on budget and resource planning.
• Develop knowledge of community resources and connect patients to supports.
• Plan in-service training and continuing education for the team.
• Contribute to quality improvement initiatives and patient care enhancements.
• Perform other duties as required for patient services.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
