Job summary

Premier Home Health Providers Inc. seeks a Medical Social Worker to lead the social services team in delivering compassionate home-based care.

You will mentor staff, oversee assessments, and ensure compliant documentation while coordinating with clinical providers to support patients in their transition home. The role is per diem with a pay rate of $100–$120 per visit; a Master of Social Work and an active California LCSW license are required along with 3+ years of management experience in home

Qualifications

• Master of Social Work (MSW) from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and relationship-building skills.

Responsibilities

• Lead daily operations of the social services team, including policies and care coordination.
• Recruit, onboard, and mentor social workers with clinical supervision.
• Oversee assessment, planning, and evaluation of social services care with documentation to meet state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build strong knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.
