Job summary

Premier Home Health Providers Inc. seeks an experienced Medical Social Worker (LCSW) to lead our social services team in CA. Per diem position with a rate of $100–$120 per visit, focusing on care coordination, staff mentorship, and quality of patient outcomes.

The role partners with leadership to manage budgets and improve community resources access. Ideal candidates hold an MSW, an active CA LCSW license, and a minimum of three years of management experience in home health.

Responsibilities

• Lead daily operations of the social services team, including policies and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers, providing supervision and professional growth.
• Oversee assessment, planning, and evaluation of social services care and ensure documentation meets standards.
• Partner with the Executive Director on the department budget and resource planning.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and continuing education for the team.
• Contribute to quality improvement initiatives and identify improvements for patient care.
• Perform other duties as required for patient services.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
