Job summary

Caring Hand Home Health Solutions, Inc. is seeking a Medical Social Worker Manager to lead the social services team in home settings and mentor staff. You will oversee care coordination and ensure compliant documentation across the organization.

The role requires an MSW and active CA LCSW license, with at least 3 years of management experience in home health. Per diem position with a rate of $100–$120 per visit, offering flexibility and growth opportunities.

Qualifications

• Master of Social Work (MSW) from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services
