Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker (per diem) to lead our home-based social services team and ensure high-quality patient care. You will mentor staff, supervise practice, and coordinate with clinical providers to support patients returning home.

Must hold an MSW and California LCSW license, with at least 3 years of management experience in home care. Flexible scheduling and opportunities to convert to full-time are included.

Qualifications

• MSW degree from an accredited program.
• Active California LCSW license.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.

Responsibilities

• Lead daily operations of the social services team and ensure care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers with clinical supervision and growth support.
• Oversee assessment, planning, and evaluation of social services; ensure proper documentation.
• Partner with the Executive Director on budgeting and resource planning.
• Build knowledge of community resources and connect patients and referral sources to support.

Please note that this role is per diem with a rate of $100 - $120 per visit.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
