Job summary

Premier Home Health Providers Inc. seeks a Medical Social Worker (per diem) in California to lead our social services team and mentor staff as we support patients at home during recovery.

You will oversee care coordination, assessments, and documentation to ensure high-quality, compliant service. Work with the Executive Director on budgeting and resource planning, build community resources, coordinate training, and help drive continuous quality improvement.

Qualifications

• MSW from an accredited program, active California LCSW license.
• 3+ years of management experience, preferably in home health.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and relationship-building skills.

Responsibilities

• Lead daily operations of the social services team with policies and care coordination.
• Recruit, onboard, and mentor social workers; provide clinical supervision.
• Oversee assessment, planning, and evaluation of social services and ensure compliant documentation.
• Partner with the Executive Director on annual department budget and resources.
• Build knowledge of community resources and connect patients to right support.
• Plan in-service training and continuing education for the team.
• Contribute to quality improvement initiatives and identify new service improvements.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
