Job summary

Best Home Health Providers Inc. seeks a Medical Social Worker in a per diem, leadership-track role in California. You will lead daily operations of the social services team, mentor staff, and ensure care aligns with ACHC standards.

You’ll collaborate with the Executive Director on budget planning, build community resource networks, and drive quality improvements while maintaining patient-centered care across our Bay Area service area.

Qualifications

• Master of Social Work from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and business operations.
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community.
• A self-directed, organized, and flexible approach—you take initiative and work well with others.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth.
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build strong knowledge of community resources and connect patients and referral sources to the right support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.
