Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker to lead our home health social services team in California. You will mentor staff, oversee assessments, care planning, and ensure regulatory compliance while coordinating with clinical partners to support patients after discharge.

The role is per diem, with a rate of $100–$120 per visit, and offers opportunities to convert to full-time based on performance. Strong leadership and communication are essential.

Qualifications

• Master of Social Work from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and interpersonal skills.
• Self-directed, organized and flexible.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures and care coordination with other clinical providers.
• Recruit, onboard and mentor social workers, providing supervision and supporting professional growth.
• Oversee assessment, planning and evaluation of social services care and ensure documentation meets state and ACHC standards.
• Partner with the Executive Director on the annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referral sources to support.
• Plan in-service training and continuing education to keep the team’s skills sharp.
• Contribute to quality improvement initiatives and identify ways to better serve patients.
• Perform other duties as required for patient services.
