Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker (per diem) in California to guide patients transitioning home, mentor a team of social workers, and uphold high-quality service.

You will oversee daily operations, ensure compliance with ACHC and state rules, and collaborate with leadership. Per visit pay ranges $100-$120.

Qualifications

• Master of Social Work (MSW) from an accredited program.
• Active California license as an LCSW.
• At least 3 years of management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and operations.
• Excellent communication and relationship-building with patients, families, staff, and community.
• Self-directed, organized, and flexible with initiative.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers with clinical supervision and ongoing growth.
• Oversee assessment, planning, and evaluation of social services and ensure compliant documentation.
• Partner with the Executive Director on department budgeting and resource planning.
• Develop knowledge of community resources and connect patients and referral sources to needed support.
• Plan in-service training and continuing education for team skill maintenance.
• Contribute to quality improvement initiatives and seek new ways to better serve patients.
• Perform other duties as required for patient services.

• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services
