Job summary

Premier Home Health Providers Inc. is seeking a Medical Social Worker with MSW and active California LCSW license to lead our social services team in delivering high-quality home health care.

Per diem position with a compensation of $100–$120 per visit. Responsibilities include supervision, care coordination, and program development to support patients and families in their transition home.

Qualifications

• MSW from an accredited program.
• Active California LCSW license.
• At least 3 years of management/ leadership experience in home care.
• Strong knowledge of healthcare regulations and ACHC standards.
• Excellent communication and relationship-building with patients, families, staff.

Responsibilities

• Lead daily operations of the social services team, including policies and care coordination.
• Recruit, onboard, and mentor social workers; provide clinical supervision and growth opportunities.
• Oversee assessment, planning, and evaluation of social services and ensure compliant documentation.
• Collaborate with Executive Director on annual department budget and resources.
• Build knowledge of community resources and connect patients and referrals to support.
• Plan in-service training and continuing education to keep skills sharp.
• Contribute to quality improvement initiatives and patient care improvements.

Responsibilities

• Lead daily operations of our social services team, including policies, procedures, and care coordination with other clinical providers
• Recruit, onboard, and mentor social workers—providing clinical supervision, observing practice, and supporting their professional growth
• Oversee assessment, planning, and evaluation of social services care, and ensure clinical documentation meets state, federal, and ACHC standards
• Partner with the Executive Director on the annual department budget and resource planning
• Build strong knowledge of community resources and connect patients and referral sources to the right support
• Plan in-service training and continuing education to keep the team’s skills sharp
• Contribute to quality improvement initiatives and help identify new ways to better serve our patients
• Perform other duties as required for patient services

• A Master of Social Work (MSW) from an accredited program
• An active California license as an LCSW
• At least 3 years of management experience, preferably in home care
• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
• A self-directed, organized, and flexible approach—you take initiative and work well with others
