Job summary

Caring Hand Home Health Solutions, Inc. is seeking a per diem Medical Social Worker (MSW) in California to lead our social services team and provide patient-centered care in home settings.

You will recruit and mentor social workers, supervise clinical practice, and ensure documentation meets state, federal, and ACHC standards while collaborating with the Executive Director on budget and resource planning.

Qualifications

• MSW from an accredited program.
• Active California LCSW license required.
• Minimum 3 years management experience, preferably in home care.
• Strong knowledge of healthcare regulations, ACHC standards, and operations.
• Excellent communication skills and ability to build trust with patients and families.
• Self-directed, organized and flexible; proactive team leader.

Responsibilities

• Lead daily operations of the social services team, including policies, procedures, and care coordination with other clinical providers.
• Recruit, onboard, and mentor social workers with clinical supervision and professional development.
• Oversee assessment, planning, and evaluation of social services care ensuring state and ACHC standards.
• Partner with Executive Director on annual department budget and resource planning.
• Build knowledge of community resources and connect patients and referral sources to support.
• Plan in-service training and continuing education to keep team skills current.
• Contribute to quality improvement initiatives and patient care enhancements.
• Perform other duties as required for patient services.

Please note that this role is per diem with a rate of $100 - $120 per visit.

• A Master of Social Work (MSW) from an accredited program

• Strong knowledge of healthcare regulations, ACHC standards, and business operations
• Excellent communication skills and the ability to build trust with patients, families, staff, and the community
