Job summary

Humana is seeking a Care Coordinator to support Illinois Medicaid members under the Illinois HealthChoice program. You will assess member needs, develop care plans, and connect families with resources to promote optimum health.

The role involves field visits and telephonic assessments, with travel primarily within the catchment area. Requirements include a relevant bachelor’s degree or licensure (LPN/LPC/LSW) and 1+ year health care experience.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• Unrestricted LPN in Illinois with 1 year experience for elderly care.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in homes or facilities; 75-90% travel.
• Assess care needs and develop interventions.
• Coordinate services and monitor the care plan.
• Assist with grievances and appeals and provide information on services.
