Humana is seeking a Care Coordinator to support Illinois Medicaid Members in the MLTSS/LTSS programs. You will assess needs, build personalized care plans, and connect families with community resources, with 75-90% local travel to member homes and facilities.

Reporting to the Manager of Care Management, you will coordinate services, monitor progress, and help members navigate benefits. Residency in Illinois and 1+ year healthcare/case management experience are required; Monday–Friday, 40 hours.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• LPN license in Illinois with 1+ year experience in assessments.
• Licensed Professional Counselor or Licensed Social Worker.
• 1+ year healthcare/case management experience.

Responsabilités

• Visit Medicaid members in homes or care settings and travel 75-90%.
• Assess needs and develop personalized care plans.
• Identify and resolve barriers to care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the case management plan.
• Ensure progress toward outcomes and address enrollee needs.
• Assist with grievances and appeals as needed.
• Provide information about services, benefits, and community resources.
• Guide families to appropriate resources and payer coordination.
