Job summary

Humana Inc. is seeking a Care Coordinator in Illinois to support Medicaid members under the HealthChoice program. You will assess needs, develop personalized care plans, and connect members with resources to improve health and well-being.

The role involves 75–90% local travel to member homes, facilities, and other care settings. You will work under a Manager of Care Management, with responsibilities including care plan monitoring, addressing barriers, and coordinating services to achieve optimal

Qualifications

• Bachelor's degree or equivalent in social sciences, social work, human services, or related field.
• LPN in Illinois with one year experience in assessments and services for the elderly.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).
• 1+ years healthcare or case management experience.
• 1+ years experience with Medicare/Medicaid or long-term care settings.

Responsibilities

• Visit Medicaid members in homes, facilities, or care settings—75%-90% local travel.
• Assess care needs to ensure services in least restrictive setting.
• Identify and resolve barriers to care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the case management plan toward member goals.
• Monitor patient care to ensure progress toward outcomes.
• Assess enrollee needs via telephonic and face-to-face assessments.
• Assist enrollee to address gaps in care and access.
• Assist with grievances and appeals.
• Provide information on MCO requirements, services, and benefits.
• Refer enrollees to community resources.
