Qualifications

• Proficiency with electronic information applications and EMRs.

Responsibilities

• Visit Medicaid enrollees in homes or care facilities to assess needs.
• Proactively outreach and complete Health Risk Assessments (HRAs).
• Identify barriers and coordinate services to support care plans.
• Monitor progress and adjust care plans to meet goals.
• Assist with grievances and appeals and provide program information.
