Job summary

Humana is seeking a Care Coordinator to support Illinois Medicaid Members within the catchment area. You will assess needs, develop personalized plans, and connect families with resources to improve well-being.

The role involves 75-90% local travel and field-based work, with schedules Monday–Friday and occasional flexibility. Reporting to the Manager, Care Management, you will coordinate services, monitor progress, and assist with grievances and appeals.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• LPN license in Illinois with 1 year experience in comprehensive assessments and elder care.
• LPC or LSW licensure required.
• Proficiency with Word, Excel, Outlook; experience with EMR/electronic records.

Responsibilities

• Visit Medicaid members in homes and facilities with 75-90% local travel.
• Assess care needs and create personalized care plans.
• Coordinate services and monitor progress against goals.
• Assist enrollee with grievances and appeals processes.
• Provide information on MCO requirements, services, and benefits.
• Guide families to community resources and supports.
