Job summary

Humana invites applications for a Care Coordinator in Illinois. You will assess patient needs, develop personalized care plans, and connect members with resources to improve health outcomes in a flexible, remote field role.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• Unrestricted Licensed Practical Nurse (LPN) in the state of Illinois with one year experience.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities and other care settings with high travel.
• Assess member needs to ensure services in least restrictive setting.
• Identify and resolve barriers hindering care.
• Plan and implement interventions to meet care needs.
• Coordinate services, monitor, and evaluate the care plan against goals.
• Monitor patient care to ensure progress toward outcomes.
• Assess enrollees' needs via telephonic and face-to-face evaluations, coordinating services.
• Assist enrollees to remediate gaps in care and access.
• Assist enrollees with grievances and appeals.
• Provide enrollee information on MCO requirements, services, and benefits.
• Refer to community resources and guide families to appropriate care resources.

Education

Bachelor's degree in social sciences, social work, human services, or a related field
