Job summary

Humana is hiring a Care Coordinator for Illinois Medicaid Members, focusing on assessing needs, developing care plans, and connecting families with resources. The role supports Humana's Illinois HealthChoice Medicaid Managed Care program and reports to the Manager, Care Management.

Time is split between home visits and community settings, with travel 75-90%. Must reside in Illinois and hold applicable licenses (LPN/LPC/LSW) and relevant healthcare experience.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• Unrestricted LPN license in Illinois with 1 year of experience in assessments and elderly services.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in homes or facilities and other settings; travel 75-90% locally.
• Assess care needs to ensure services in least restrictive settings.
• Identify and resolve barriers hindering care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the care management plan against personal goals.
• Monitor patient care to ensure progress toward outcomes.
• Assess enrollee needs via telephonic and face-to-face assessments.
• Assist enrollee in remediating gaps in care and access.
• Assist enrollee with grievances and appeals.
• Provide information on MCO requirements, services, and benefits.
• Refer enrollees to community resources.
