Job summary

Humana is seeking a Care Coordinator in Illinois to support the Medicaid HealthChoice program. You will assess members, develop personalized care plans, and connect families with resources to improve health outcomes. This field role involves 75–90% local travel and close collaboration with care teams.

Reporting to the Manager, Care Management, you will work to ensure members receive appropriate services, navigate the system, and address gaps in care. Flexible scheduling may apply.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or a related field.
• Unrestricted LPN in Illinois with 1 year experience conducting comprehensive assessments and provisioning services for the elderly.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsibilities

• Visit Medicaid members in homes, Assisted Living Facilities, and Long-Term Care Facilities; 75-90% travel.
• Assess care needs to ensure least restrictive settings and optimal well-being.
• Identify and resolve barriers hindering effective care.
• Plan and implement interventions to meet care needs.
• Coordinate services and monitor the care management plan.
• Monitor progress toward outcomes through assessments and care planning.
• Assess enrollee needs via telephonic and face-to-face evaluations.
• Assist enrollees to remediate gaps in care and access.
• Assist enrollees with grievances and appeals.
• Provide information on MCO requirements, services, and benefits.
• Refer enrollees to community resources.
• Guide members to resources and coordinate with providers.

Education

Bachelor's degree in social sciences, social work, human services, or a related field

Unrestricted Licensed Practical Nurse (LPN) in Illinois with 1 year experience

Licensed Professional Counselor (LPC)
