Qualifications

• Bachelor's degree in social sciences, social work, or related field.
• LPN license in Illinois with 1 year of assessments experience.
• Licensed Professional Counselor (LPC) credential.
• Licensed Social Worker (LSW) credential.

Responsibilities

• Visit Medicaid members in their homes or care facilities, with 75–90% travel.
• Assess needs and develop personalized care plans.
• Identify and resolve barriers to care.
• Coordinate services and monitor the care plan.
• Assist with grievances and appeals.
• Provide information on MCO requirements, services and benefits.
• Refer enrollees to community resources.
