Job summary

Humana in Illinois is seeking a Care Coordinator to support the Illinois HealthChoice Medicaid Managed Care program. You will assess member needs, develop individualized care plans, and connect individuals and families with resources to help them thrive.

This field position reports to the Manager, Care Management with 75-90% local travel. The role requires a bachelor’s degree in a related field and licensure (LPN in Illinois, LPC, LSW) with experience in health care, case management, and EMR

Qualifications

Bachelor's degree in social sciences, social work, human services, or a related field.
Unrestricted LPN in Illinois with one year of experience conducting comprehensive assessments and formal service delivery for the elderly.
Licensed Professional Counselor (LPC).
Licensed Social Worker (LSW).

Responsibilities

Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities; 75-90% local travel.
Assess needs and develop care plans to connect members with appropriate services.
Identify and resolve barriers hindering effective care.
Coordinate services and monitor the case management plan toward personal goals.
Assist enrollees with grievances, appeals, and accessing community resources.
