Job summary

PACS is seeking a Nurse Manager with expertise in PDPM and MDS to oversee the facility's assessment processes and plan of care. The role focuses on coordinating MDS submissions, supervising staff, and ensuring regulatory compliance across all care levels.

The ideal candidate has a BSN, active RN licensure, 3+ years of supervisory LTAC experience, and proficiency with Microsoft Office and PCC. This on-site role offers opportunities for leadership and process improvement.

Qualifications

• BSN preferred and RN licensure in good standing.
• Minimum 3 years supervisory experience in healthcare.
• Long Term Care experience preferred.
• Proficiency with Microsoft Suite and PCC software.

Responsibilities

• Oversee facility's weekly PDPM meetings for skilled patients.
• Track utilization of PDPM tools and related metrics.
• Monitor QM/QASP performance; provide data analysis and recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant changes.
• Communicate level of care to all disciplines.
• Coordinate MDS participation within regulatory time frames; ensure documentation is complete.
• Maintain accurate MDS schedule with correct reference dates.
• Enter and submit MDS data electronically; verify submissions and correct as needed.
• Coordinate interdisciplinary MDS activity and resident care conferences; ensure reviews are completed.
• Assist in formulating and revising care plans; set priorities and nursing interventions.
• Evaluate care plans for comprehensiveness and individuality.
• Assess achievement of outcomes and revise plans as needed.
• Manage levels of care changes; notify affected departments.
• Generate and transmit acuity change forms to the appropriate agency.
• Maintain confidentiality of resident care information.
