Job summary

PACS Group is seeking an experienced nursing leader to oversee the facility's assessment processes, ensuring PDPM/MDS activities are conducted per guidelines and that resident care plans reflect accurate levels of care.

The role includes coordinating weekly PDPM meetings, monitoring QM/QASP performance, and delivering PDPM/MDS training to new coordinators, with emphasis on compliant documentation and timely care planning.

Qualifications

• BSN preferred
• Good standing with State Board of Nursing
• Continuing education/licensing requirements
• 3 years of supervisory experience preferred
• Long Term Care experience preferred

Responsibilities

• Oversees facility's weekly PDPM meetings for skilled patients
• Tracks utilization of PDPM tools and related processes
• Monitors QM and QASP performance monthly/quarterly with data analysis and recommendations
• Provides PDPM and MDS training to new MDS Coordinators
• Assesses health status and level of care for all new admissions
• Ensures timely completion of all MDS assessments
• Maintains documentation and coordinates MDS scheduling and reviews
• Verifies electronic submissions of MDS and maintains records
• Maintains confidentiality of resident care information
