PACS is seeking a dedicated Nursing Supervisor to oversee the facility’s resident assessment processes, including MDS and PDPM activities. You will coordinate interdisciplinary teams, ensure timely MDS submissions, and monitor care plans to align with regulatory standards.

The role requires a BSN, active State Board of Nursing standing, and prior supervisory experience, preferably in Long-Term Care. Proficiency with Microsoft Office and PCC is expected; travel may be required.

Formación

• Bachelor of Science in Nursing (BSN) preferred; must be in good standing with State Board of Nursing and maintain continuing education/licensing requirements.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.

Responsabilidades

• Oversee facility weekly PDPM meetings for skilled patients.
• Track utilization of PDPM tools (PDPM, CSR, weekly tracking tools).
• Analyze QM and QASP performance; provide data-driven recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions; ensure timely MDS completion.
• Coordinate MDS across disciplines within regulatory time frames.
